Description & Services
Description (Service) Occupational therapist will provide wheelchair/seating assessments, prescriptions and modifications, as well as assist patients in exploring wheelchair funding options.
Hours Mon-Sun 8:30 am-8:30 pm
Eligibility Ages: 18 year(s) and up 
 
Patients must have a valid OHIP card * patient must be able to get in and out of their house and live within 30-45 minutes of clinic * patient is independent with transfers or requires only one person to assist with transfers * seating/positioning is identified as patient's primary need * patient is in need of wheelchair/seating adjustment or replacement and their needs cannot be met through a community equipment vendor
