| Application | Referrals can be made through Here 24/7 or directly to the ACT team. A referral form can be downloaded form the Thresholds Homes and Supports website. There are two ACT teams serving the Waterloo Region. One is run through the Grand River Hospital and one through Thresholds Homes and Supports. The two teams have joint intake process. The form once completed can be sent to Here 24/7 or either ACT team. |
