|Description (Service)||Multi-disciplinary team that provides intensive case management focusing on support, rehabilitation and treatment to individuals with severe and persistent mental health issues. The program helps consumers achieve their goals, improve their health and quality of life, and live as independently as possible. Team members’ specialties include psychiatry, registered nursing, social work, occupational therapy, addictions, therapeutic recreation and peer support|
|Hours||8 am-8 pm on business days * 8:30 am-12 noon on weekends and public holidays|
|Eligibility||Ages: 18 year(s) and up
Individuals with severe and persistent mental health issues, who have tried other intensive case management services but need an even more intensive service.
|Application||Referrals can be made through Here 24/7 or directly to the ACT team. A referral form can be downloaded form the Thresholds Homes and Supports website.
There are two ACT teams serving the Waterloo Region. One is run through the Grand River Hospital and one through Thresholds Homes and Supports. The two teams have joint intake process. The form once completed can be sent to Here 24/7 or either ACT team.
