Description (Service) The Community Diabetes Program Waterloo Region has 3 main sites and many satellite sites to provide education and support to adults with diabetes. Individual consults with a nurse, dietitian, exercise specialist or foot specialist are available as are group sessions. Foot and eye screening to detect diabetes related disease are available at no charge to our patients. Our goal is to support patients and their families with education and tools that help them to maximize diabetes self-management and improve their health and wellbeing.
Hours Mon, Wed and Thu 8:30 am-8 pm * Tue and Fri 8:30 am-4:30 pm
Eligibility Ages: 18 year(s) and up 
 
Adults with Diabetes, pre-diabetes or at risk/ family members, caregivers and support persons as well.
Application Patients can be referred by a health care provider or they can self-refer. Referral forms can be found at
