Description (Service) |
The Community Diabetes Program Waterloo Region has 3 main sites and many satellite sites to provide education and support to adults with diabetes.
- Individual consults with a nurse, dietitian, exercise specialist or foot specialist are available as are group sessions.
- Foot and eye screening to detect diabetes related disease are available at no charge to patients.
- Goal is to support patients and their families with education and tools that help them to maximize diabetes self-management and improve their health and wellbeing.

Ages: 18 year(s) and up Adults with Diabetes, pre-diabetes or at risk/ family members, caregivers and support persons as well. |
