Description (Service) The Community Diabetes Program Waterloo Region has 3 main sites and many satellite sites to provide education and support to adults with diabetes. 
     
  • Individual consults with a nurse, dietitian, exercise specialist or foot specialist are available as are group sessions. 
  • Foot and eye screening to detect diabetes related disease are available at no charge to patients.  
  • Goal is to support patients and their families with education and tools that help them to maximize diabetes self-management and improve their health and wellbeing. 
Hours Mon, Wed and Thu 8:30 am-8 pm * Tue and Fri 8:30 am-4:30 pm
Eligibility Ages: 18 year(s) and up 
 
Adults with Diabetes, pre-diabetes or at risk/ family members, caregivers and support persons as well.
Application Patients can be referred by a health care provider or they can self-refer. Referral forms can be found at www.waterloowellingtondiabetes.ca
