| Description (Service) | Community Support Coordination is a service provided to individuals who require mental health supports but do not require direct housing assistance from Thresholds. The service offers individualized supports with a strength based focus. Emphasis is placed on working with people to coordinate services that they receive as well as focusing on individual recovery goals, maximising independence and improving wellness and quality of life. |
