Waterloo Wellington Diabetes, hosted by Langs in Cambridge Ontario, offers a central intake for all referrals to diabetes education and specialists. This means improving access to diabetes care by finding the right care at the right time at the right place for individuals and families with diabetes. Mentoring program supports health service providers throughout the region to deliver high quality, evidence-based care in diabetes. |

Individuals who are at high risk for diabetes or who have prediabetes, type 1, or type 2 diabetes, or health care providers working with individuals with diabetes. |
