Description (Service) Provides support to housing for persons with problematic substance use who are involved in or have completed an addiction treatment program, and who are also homeless, at risk of becoming homeless, or who are considered inadequately housed.
Hours Mon-Sun 24 hours
Eligibility Ages: 18 year(s) and up 
Individuals must: 
  • Be at least 18 years of age 
  • Have recent involvement in or have completed an addiction treatment program 
  • Have recent experience with homelessness or at risk of homelessness (including transitional home, living on the street) 
  • Have history of or are currently dealing with chronic and/or problematic substance use concerns 
  • Frequent use of withdrawal management and other addiction treatment services 
  • Frequent use of emergency rooms and contact with the criminal justice system 
  • May have concurrent mental health diagnosis 
  • Committed to active participation in case management and support services (must want support through the length of the program)
